Madison Township, PA — The Brookville community is grieving the sudden loss of Samantha Lyn “Sami” Hetrick, 21, who tragically died following a UTV accident in Madison Township, Armstrong County, in the early morning hours of Sunday, August 31, 2025.
According to Pennsylvania State Police at Kittanning, troopers responded around 2:27 a.m. to the 900 block of Deanville Road for a reported ATV crash on private property. Authorities said the UTV was operated by Chase Bish, with Samantha riding as a passenger. She sustained severe injuries and later succumbed to them. Armstrong County Coroner Brian K. Myers confirmed he was called to the scene and reported that an autopsy will be conducted.
Remembering Samantha
Born February 19, 2004, Samantha was the cherished daughter of Daryl Hetrick and Jamie Bennett Smith. A 2022 graduate of Brookville High School, she was pursuing her nursing degree at PennWest Clarion, fulfilling her dream of becoming a nurse.
Sami lived a life centered on service and community. She coached gymnastics at the Brookville YMCA, provided private home health care, and was a member of the Brookville Fireman’s Club. She loved the outdoors—hunting, boating, running—and was a devoted dog mom to Finnigan, Hank, Bailey, and Molly.
Most of all, Samantha treasured time with her family, finding joy in gatherings, celebrations, and holidays.
Survived by a Loving Family
She is survived by her parents, step-parents, siblings, grandparents, and a large extended family who will forever cherish her memory. She was preceded in death by several great-grandparents.
Funeral Services
Visitation will be held Friday, September 5, 2025, from 2:00–4:00 p.m. and 7:00–9:00 p.m. at Shugarts Funeral Home Inc., Brockway. A Funeral Service will follow on Saturday, September 6, 2025, at 11:00 a.m., with Pastor Joni Williams officiating. Interment will take place at Riggs Cemetery in Richardsville.
