Tyler Woman Killed, Driver Arrested for Intoxication Manslaughter After Crash in Smith County

SMITH COUNTY, Texas — A tragic crash in the Winona area early Wednesday morning left one woman dead and another driver facing serious criminal charges.

According to the Texas Department of Public Safety (DPS), the fatal wreck occurred on FM 2015 north of County Road 345 at approximately 7:00 a.m. on October 15, 2025. Investigators determined that a vehicle driven by 31-year-old Jeremiah Munoz of Tyler was traveling northbound when it crossed into the southbound lane, colliding head-on with another vehicle driven by 35-year-old Brandi Carter, also of Tyler.

The impact caused significant damage to both vehicles. Emergency crews arrived quickly, but Brandi Carter was pronounced dead at the scene due to injuries sustained in the crash.

Troopers at the scene reported that Munoz showed signs of intoxication, and following a field investigation, he was taken into custody. He has been charged with intoxication manslaughter and booked into the Smith County Jail.

Authorities closed a portion of FM 2015 for several hours as they conducted a detailed crash reconstruction and collected evidence.

The Texas Department of Public Safety continues to investigate the crash to determine the exact sequence of events that led to this devastating incident.
